Registered Nurse Navigator Position Summary

As an EP Nurse Navigator at our cardiology outpatient clinic, you will play a crucial role in managing a diverse caseload of patients and guiding them through the complexities of healthcare services. Environment and Shift details :

This role is full-time, Monday – Friday—closed weekends and holidays.

Registered Nurse Navigator Required Qualifications:

• Graduation from an accredited BSN or RN program.
• Proficient in applying advanced clinical standards of care, organizational policies, and procedures in treatment settings.
• Solid understanding of payer standards, guidelines, and regulatory requirements.
• Comprehensive knowledge of developmental stages across the lifespan, with the ability to deliver age-appropriate care.
• Experienced in recognizing signs and symptoms of suspected abuse and familiar with the corresponding reporting mechanisms.
• Strong documentation and communication skills, enabling effective collaboration with providers and treatment teams regarding patient care plans.
• Proficient in conducting thorough nursing assessments.

Registered Nurse Navigator Responsibilities

• Screen and identify at-risk patients by assessing their medical, financial, or social needs and guide them through appropriate care plans and resources.
• Analyze patient care data to make recommendations and referrals, ensuring optimal care coordination and advocacy during complex care conferences.
• Facilitate access to healthcare services, assisting with referrals to disciplines such as nutrition, social work, and community resources.
• Collaborate with healthcare teams to track interventions and patient outcomes, developing policies and training based on best practices identified through data analysis.
• Provide targeted educational programs and materials to educate patients, families, and healthcare team members on relevant health issues and care strategies.
• Act as a patient advocate, ensuring that patients' rights and dignity are respected and their psychosocial and emotional needs are addressed.
• Manage resources effectively to support fiscal responsibility and high-quality patient care, participating in developing and reviewing program standards and guidelines.

Registered Nurse Navigator Pay and Benefit Expectations

Cardiac Study Center/ Pulse Heart Institute provides a comprehensive benefits package, including competitive salary, medical, dental, and retirement benefits, and paid time off. As various pay transparency laws require, CSC/ PHI shares a competitive compensation range for candidates hired into each position. The starting pay for this position is $36.32 , and the pay scale is $36.32 - $54.83 USD . However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
